If you are not enrolled, you will not be able to sign in. WHAT YOU CAN SEE Home shows your next visit, who it is with, and how many more are scheduled after it. It also shows your care advocate and the ways they can help, from finding earlier appointments to insurance, bills and coordinating between your doctors. Care plan shows what your advocate has written down: your situation, your medical history, what gets in the way of your care, your goals, and what the team recommends.
